All athletes must have a current Special Olympics Texas medical realease form on file before competing in an Area SOTX Competition.
Athlete medicals are due by September 14th, 2023. Please send completed medicals to Gretchen Reeves at greeves@sotx.org. Blank athlete medicals can be found by clicking here. Incomplete medicals will not be accepted. Please make sure all 7 pages of the medical have been completed before sending them to Gretchen.
