We want to expand our Healthy Athletes program to each of the areas. The first step is bringing MedFest to the areas, to provide free health screenings that end with a completed SOTX Athlete Medical form, which is good for three years. But we need help – we can’t provide these free health screenings without qualified health professionals (licensed physicians, physician assistants and nurse practitioners). If you know someone who fits these qualifications, will you please ask if they would get involved? For more information, please email Norm Arias, Director of Field Services. 

NOTE:  Pages 1-3 of the athlete medical form must be completed and signed by the parent/guardian before the athlete will be screened.  If the athlete is his/her own guardian, he/she can complete pages 1-3 and sign for themselves.
