To enroll to become an athlete, complete an Athlete Enrollment/Medical Release Form.
After completing the Athlete Enrollment/Medical Release Form, submit them to your local area director. (Spanish version of Athlete Medical Form - Only for use in translating the English Athlete Medical Form)
